I have had several requests for the LOA template which is referenced in Appendix 10 of the SMEX Guidelines.

Here is a sample for a Phase A study. You, of course, will have to put in appropriate activities and responsibilities for H/W development and operations (i.e. Phase B/C/D/E). But this shows a format that is acceptable.

I will also have this template placed in the SMEX Library.
